How To Register and Schedule

• The Registration button below will indicate what forms to complete for a Consultation verses an Evaluation + Session.

• After Registration is complete, use the Appointment tab to book using our online calendar. Appointments are via Zoom video.

• For Consultations (allow 45-55 minutes)

• For Evaluation + Session (allow 90 minutes)

IMPORTANT

• Only schedule appointments once the proper Registration forms have been completed.

• Be Certain to Keep the Consultation appointment for it is only offered once and is without a reschedule option.

• The only next option is the Evaluation + Session.
