How To Register and Schedule

The Registration button below will indicate what forms to complete for a Consultation verses an Evaluation + Session.

After Registration is complete, use the Appointment tab to book using our online calendar. Appointments are via Zoom video.

For Consultations (allow 20-25 minutes)

For Evaluation + Session (allow 90 +/-minutes)

IMPORTANT

Only schedule appointments once the proper Registration forms have been completed.

Be Certain to Keep the Consultation appointment for it is only offered once and is without a reschedule option.

The only next option is the Evaluation + Session.
